چکیده: Abstract: The need for the elderly and infirm to receive care and medical services at home, as well as the high cost of hospital care, has increased attention to home care services in recent years. On the other hand, the transportation system used in these services has led to the production of air pollutant gases such as carbon dioxide, and this requires attention to reduce the emission of these pollutants by the transportation system for environmental considerations. In this study, a home care service system with multiple centers (including medical centers and laboratories) is considered. In this system, nurses should be sent to provide services to patients.The goal is to determine the optimal route for nurses, so that the costs of the transportation system and the emission of air pollutants are minimized. To get closer to the real world, each patient's demand is considered fuzzy indefinitely. In order to deal with uncertainty, the Jimenez method, which is computationally efficient, maintains linearity and does not increase objective functions and constraints, has been used. Small and meta-heuristic algorithms(NSGA-II and PESA-II) have been used to solve it in larger sizes. The results of solving the routing model have been analyzed and also the effects of parameter changes on the values of the objective function have been investigated
